Your child is being asked to be a part of our distict's California Healthy Kids Survey (CHKS), sponsored by the California Department of Education. This is a very important survey that will help promote better health among our youth and combat problems such as drug abuse and violence. Please read this form for information about the survey and for instructions on how to withdraw your child. If you do not want your child to complete the survey, you must notify your school.

Se le está pidiendo a su hijo que participe en la Encuesta de Niños Sanos (CHKS, por sus siglas en inglés) del distrito escolar, patrocinada por el Departamento do Educación de California. Ésta es una encuesta de suma importancia que fomentará una mejor salud entre nuestros jóvenes y luchar contra problemas tales como el consumo do drogas y la violencia.
